There have been some incredible midwife-led case studies over the years (Neighbourhood Midwives, One-to-One, The Albany Practice) with excellent results that have not survived wider NHS systemic issues. How can we learn from, and implement, the winning ingredients from these cases?
Other organisational models: Buurtzorg is an example of an organisational system that puts the needs of people first. We are exploring whether Buurtzorg principles could be applied to UK Midwifery to enable midwives to operate with independence in a flat organisation structure.
Based in the Netherlands Buurtzorg has a unique self-management structure where small teams of community nurses self-manage nearly all aspects of their work. There are no managers - instead, a centralised curriculum with a small ‘back office’ admin staff and ‘coaches’ that can step in to support if teams get stuck. The communication structure allows teams to make decisions fast and not get paralysed by ‘group think’.
